If you need to make a claim

Our claims process is fast and easy! Just follow these simple steps:

Step 1 - Download a claim form - Many veterinarians also have them available right at their office.

Step 2 - Take your pet to any licensed veterinarian for diagnosis and treatment.

Step 3 - Pay your veterinary bill in full, fill out sections 1 and 4 of the claim form, and have your veterinarian complete sections 2 and 3.  Remember to sign your form!

Step 4 - Attach your receipt(s) or original invoice to the claim form. Be sure to keep a copy for your own records.

Step 5 - Mail or fax your claim form to:

You're done! - Watch for your cheque in the mail. Don't worry - we process complete claims in an average of 5 business days.
